INSTRUCTIONS FOR GIVING
TO THE SHADE MINISTRY BY MAIL:
1. Write a note on a piece of paper or sticky note that says “Account #205” .
2. Make the check payable to “Mission Quest”, with
“acct. #205” in the memo line. (Please DO NOT write ouro name anywhere on the check.) **
3. Place in an envelope with the check along with the separate note.

** The memo line is important for your IRS records and to direct the funds to the proper account.
Your contribution to Mission Quest is tax deductible to the extent allowed by law. Per IRS guidelines, while every effort will be made to apply your gift according to an indicated preference, if any, Mission Quest has complete discretion and control over the use of donated funds.
